Welcome Back Students! Our classes are better than ever this year and we can't wait to share them with you. If you would like to pre-register for the upcoming classes, simply fill out the form below. There will also be flyers distributed at your school or download and print the form provided to you from the link below. You may return the completed form along with registration fee and the monthly tuition to the school office or with your child on the first day of class. Forms MUST be signed by the parent and/or guardian in order to participate in this program.

I release Amazing Feats, location facility, associates, all employees and Erica Taylor from any and all liabilities in the event of illness, accident, claims or injury. I understand my child is enrolled with Amazing Feats and I will pay tuition on or before the first of the month. There will be a mandatory late charge of $10.00 per week for payments made after the 8th. If payment is not received by the 15th, your child(ren) will be dropped and the monthly fee will still apply. There is a $25 charge for returned checks. Withdrawal of a student requires a written notice of at least one month in advance, or you will be billed the entire month. No refunds or credits for missed lessons, although make-ups are offered and recommended. Occasionally, photos may be taken during class or demonstrations. Amazing Feats reserves the right to use the photos in the newsletter and/or website for use of publicity advertising and promotions.

Withdrawal of a student requires a written notice of one month in advance.
